If useful, write a few notes. This website does not save or send your answers.
When you are under pressure, remembering questions can be difficult. These prompts may help you prepare. They are not tests or official forms, and you do not need to answer them all.
Keep notes in a secure place of your own. Share only what you choose with a person or service you select. You can seek help without writing anything down.
Before booking
What is troubling me most? A phrase or example is enough.
Who will see me? What are their name, profession and specialty? Where can I check registration and current authorisation?
Do they have relevant experience? If training, who supervises and is responsible?
Is this GeSY or private care? What will I pay, how long is the session and are there other charges?
What are the terms for cancellations, emergencies and prepaid sessions?
What would help me participate, including language, access or privacy for online sessions?
Where can I find the terms and privacy information?
Reviewing my care
What change would make a meaningful difference in my life?
How does the therapist explain my difficulties? Do I understand it, and does it fit my experience?
What do they propose, why, and what alternatives exist?
What may help, what may be difficult and what is expected between sessions?
When will we review progress?
What helps, does not help or has worsened? What one issue do I want to raise?
Who can I contact between sessions, and what should I do if urgent help is needed?
Preparing to seek advice about a concern
Who provided care? Record what you know.
What happened and approximately when? What words or actions do I remember, and what remains uncertain?
What messages, agreements or receipts do I already have? A complete file is not required to seek advice.
How did it affect me? Do I need safety, support or different care now?
What next step do I want: an explanation, information, another opinion or formal examination?
Which body can guide me? What should I ask about deadlines, cost and process?
Who can support me while I decide? If I complain, what will be shared with the professional?
