The first copy is generally free and the normal response period is one month. Some circumstances allow an extension with notice.
Understanding the request
Access concerns information about you. It does not always mean receiving every original document exactly as stored. Information about others may require specific redactions rather than refusal of the whole request.
You can write: “I would like access to and a copy of the personal data you hold about my care. Please explain how I can receive it securely.”
If the answer is unclear
Keep the request and response and ask for any restriction to be explained. You can seek advice from the relevant data-protection authority.
Access to existing information differs from commissioning a new report. You can raise factual errors, but access does not mean every professional opinion must change or every record must be deleted.
